Obtaining A Copy of an EMS Report
For Patients:
To obtain a copy of an EMS report, a Protected Health Information Release and Affidavit must be completed and submitted to the Charlotte County Fire & EMS Billing Division.

Please fax requests with release of authorization to 941-833-5630.
Charlotte County does not charge patients for obtaining their medical records. For any other authorized requests, as per Florida State Statute 456.057, Rule 64B8-10.003, we charge $1.00 per page up to 25 pages and $0.25 per page after that.
Paying an EMS Bill
We are unable to take credit card information over the phone.
Payments can be made online as follows:
- For dates of service prior to 10/1/21 use: AmbulanceBilling.com
- For dates of service after 10/1/21 use: CCFEMS.PayAmbulance.com
